Here at Jeffords Plastic Surgery, we honor our patients with a special rewards program for their continued loyalty.
Step 1: Simply fill out Section A on this form and give to a friend. No limit on number of new patient referrals!
Step 2: Your friend will fill out Section B of this form.
Step 3: Once your friend has booked a cosmetic surgery of $2500 or greater , we will provide you with 2 gift certificates by mail: $250 gift certificate towards the cost of your next cosmetic surgery of $2500 or greater & $50 gift certificate towards your next Botox/Dysport, Filler, or laser treatment.
Step 4: Don’t worry…. You haven’t been left out friend… If you book your surgery of $2500 or greater, you will receive an additional $100 discount on your cosmetic surgery when you present this completed form on your consultation day. You will also receive a $25 gift certificate toward your next Botox/Dysport, Dermal Filler, or laser treatment.
Rules &Restrictions:
- Cosmetic surgery must be a minimum of $2500 to receive certificate.
- Certificates are non-transferable and may not be redeemed for cash.
- New patients may only redeem this offer 1 time.
