Filler Referral

Here at Jeffords Plastic Surgery, we honor our patients with a special rewards program for their continued loyalty.

We have two programs, one for surgery referral and for Botox, Dysport, Fillers and Laser referral. 

We now offer a $25 gift certificate towards Botox, Dysport, Fillers & Lasers when you refer a new patient.

Step 1:  Our existing patient is to fill out Section A on the form (link below) and give to a friend, it’s that easy!!

Step 2:  Have your friend fill out Section B on the form and bring in on the day of treatment. 

After your friend receives the treatment, we will mail you your $25 gift certificate.
